Current health plan enrollees

If you received a letter to complete your Wholeness Health Plan activities, click on the links below to complete the activities.  Please use the Quick Guide below and follow the instructions carefully. 

New employees

New employees and covered spouses are encouraged to complete their wellness activities within 90 days of their health plan effective date. The Quick Guide for New Hires below will walk you through each step of the process.  Printing this guide is suggested. In addition, the video below will give you a brief introduction to the Wholeness Health Plan.

A: Assessment and Account

Complete your Health Risk Assessment (HRA).

Are you a subscriber or spouse on the health plan?

If you do not have an activation code to create a MyChart account, call the Help Desk at 1-877-558-0090. If you already have an account, you have fulfilled this requirement.

B: Biometric Screening

To make an appointment online, please click the appropriate button below and follow the instructions in the quick guide above. You may also make an appointment over the phone by calling 844-251-6524. (You may experience extended wait times when using the phone service.)  There are 2 screening options available.  Option 1:  Onsite Screening      Option 2:  Print out a voucher to visit a LabCorp location

C: Care Management

If you received a letter inviting you to participate in care management services, find the forms you need to complete. (PLEASE NOTE:  This is not a requirement for everyone.  If you have not recieved a letter for the PRIME or nicotine programs, please skip this step.)

In certain circumstances, if an employee or a spouse is unable to participate in any of these activities, such activities may be waived or reasonable alternative arrangements may be offered. To request an accommodation or waiver, please follow the appeals process as described in section VIII. F. How to File an Appeal of your health plan document.
