WELCOME TO THE LLUH EMPLOYEE HEALTH PLAN!
This is a self-insured plan sponsored by LLUH. The Plan provides comprehensive medical, dental, prescription drug and optical coverage for all benefit eligible employees and their dependents. With this transition to a new plan we would like you to have information regarding your LLUH health plan at your fingertips! Please remember, you must enroll into the Wholeness Health Plan during Open Enrollment (November 1 -15) for coverage beginning on January 1, 2019.
All expenses incurred prior to January 1, 2019, will continue to be processed under the current plans administered by Adventist Health. Any service or expense incurred January 1, 2019, or after will be covered by your new plan and any questions should be directed to the new plan Administrator, LLUH’s Department of Risk Management at (909) 651-4010 or via email at RiskManagementHealthPlans@llu.edu.
The Health Plan Document is a comprehensive written description of the operation and administration of the health plan. This plan document tells you about the benefits you are entitled to under the plan and provides guidelines to be used by the plan administrator in decision-making when it comes to plan operations.
The Schedule of Benefits is a single-page summary which provides highlights of your plan coverage and co-payments. This document will give you a quick look at your benefits under the plan.
This document lists your monthly health plan contributions for the Wholeness Health Plan with the wellness discount. Costs differ between tiers of coverage.
This health plan comparison document will highlight some of the differences and similarities between your current health plans and LLUH’s Wholeness Health Plan with a wellness discount. Please note, all Murrieta employees enrolling onto an LLUH health plan during the 2019 Open Enrollment will be able to receive the wellness discount for the Wholeness Health Plan.
