Benefits information: FMLA/leave of absence absence notice requirements

If leave is requested for a foreseeable medical treatment,
the employee must:
  • Make an effort to schedule the treatment to avoid disruption to the employer’s operations.
  • Provide the employer with at least 30-days notice before the leave is scheduled, 1 to 2 days notice for more unexpected events, and as soon as practicable in cases of emergencies.
If your intermittent FMLA includes a need to attend regularly scheduled doctor appointments or medical treatments for yourself or your family member, you will be asked to provide in advance a written appointment schedule to your manager and/or benefits specialist.
