New ‘FORMS-D’ for proposals due on or after May 25, 2016.
The NIH periodically updates grant application forms in order to remain current with the most recent form sets available through Grants.gov and approved by the Office of Management and Budget.
The forms that will likely affect you during your proposal development are:
- New “Authentication of Key Biological and/or Chemical Resources” http://grants.nih.gov/grants/guide/notice-files/NOT-OD-16-011.html
- New biographical sketch format page and clarifications: https://grants.nih.gov/grants/guide/notice-files/NOT-OD-16-080.html
- Assignment Request Form (Used to direct funding component assignment preference, study section preference, individuals who should not review your application due to conflicts, and scientific areas of expertise needed to review your application)
- New Vertebrate Animals section added http://grants.nih.gov/grants/guide/notice-files/NOT-OD-16-006.html.
- Combined Planned Enrollment Report and Cumulative Inclusion Enrollment Report forms into a single form (for human studies)
- New “Data Safety Monitoring Plan” (NIH supported Clinical Trials)
