The LLUH IRB has updated several templates on the IRB Toolbox webpage including: Protocols, Consents, PHI Authorizations and the California Experimental Subject Bill of Rights. Please note the following important changes:
The new California Experimental Subject Bill of Rights template no longer requires a signature.
The PHI Authorization template no longer requires a PI signature.
